Registration Fee Update
Due to delay in Visa release for several countries, the Early Registration Fee of $500 USD ($250 USD residents/Fellows/Students)
has been extended until June 30th, 2008.
Method of Payment
Registration fees are accepted with credit card (VISA or Mastercard only) or by check
(“US dollars” only made payable to the “University of Washington”).
The last day to preregister is June 30, 2008.
After this date, participants will have to register on-site at the symposium in Seattle.

What's Included
The fee for this symposium includes catering, all scientific materials and transcript of Category 1 credit. To ensure enrollment, please complete the enclosed registration form and forward it with the appropriate tuition.
Allow up to fifteen working days to receive your written confirmation of enrollment. Driving and parking instructions will be included with your registration confirmations. If you have not received written confirmation prior to your departure for the course, please contact the Continuing Medical Education office at +1 206 543 9496. Continuing Medical Education will not be responsible for expenses incurred by an individual who is not confirmed and for whom space is not available at the symposium.
Cancellation Policy
The Office of Continuing Medical Education must receive written notification of your cancellation.
A $100.00 processing fee will be deducted from refund requests received before June 18th.
No refunds will be issued after June 18.
